Transition Discoveries Initiative

Dear Union Families,

Union Area School District’s Special Education Transition Program has been awarded a grant from the Transition Discoveries Initiative.  This grant will assist in improving the Special Education Transition Program, as well as postsecondary education and employment for our students. Transition planning is required for all students with disabilities, at the time the student turns 14, who qualify for special education services. The transition plan includes the student’s goals in the areas of postsecondary education, employment, and independent living.  A coordinated set of activities are planned for each goal area.

The Transition Discoveries Initiative is a three-year process.  We are in Year 1, which is the data collection stage.  Please complete the parent survey and student permission form.  Students will complete the survey in their special education class.
